If you want to print out an OPARS application then fill it out by hand, please click on the Membership Form #1. Print this page out by going to the bottom of the page and clicking "Print This Page", and then fill it out by hand. Finally, submit the application to the address above with your dues.

Then fill out the form, once you are finished filling it out, go to the bottom of the page and click "Print This Page" to print your application.
